To serve you more efficiently, please bring the following items to your appointment:
- Insurance, Medicare, and/or Medicaid Cards
- Social Security Number
- Current eyeglasses, reading glasses, computer glasses and prescription sunglasses
- Current eyeglass prescription (new patients only)
- Current contact lens prescription
- List of current medications
